Writing a story that is true to the spirit of a single show (movie or book) is hard. Creating a crossover that combines elements from more than one universe is exponentially harder. Most crossovers fail, either because the author lacks sufficient understanding of one of the universes, or because they haven't paid attention to the details required to make the two universes blend.
Here are a few of my favorites.
Smallville/Justice League

"Ad Astra Per Aspera" is a chilling story. The writing isn't perfect-- there are confusing point of view shifts, and logic errors worthy of Millar and Gough. But the sheer emotional intensity of the story outweighs these flaws, and guarantees that you'll remember this tale. There is plenty of precedent in the comics universe for Superman/Batman crossovers, but we've never seen a Clark Kent quite like this.

"Alternity" by Legion starts when alternate versions of Jim and Blair step through the Stargate, anxious to pass on the knowledge they've gained in hopes that our Earth can be saved.

On vacation in England, Giles encounters a sixteen year-old Lex. Bloodied but not bowed sums this one up. A true classic.
Runaway Trains by C. L. Kamnikar

Dawn and Clark have both run away from home. They meet up in Denver, and swap stories about the strangeness of their lives. The supporting cast (Spike, Lex) add a nice touch, although these are definitely the season 1 versions of Clark and Lex.
